New Patients

How to register as a new patient:

We welcome new patients to our surgery. If you would like to see one of our doctors, please call us in advance on 01 498 6900.  

When you arrive at the surgery for the first time, you will be asked to fill out a patient registration form which we would ask you to complete as comprehensively as possible. You will then meet with one of our doctors.

It is advisable that you obtain your medical records from your previous doctor and these will be scanned into your file.

If you are applying for a Medical Card, Medical Card application forms are available at reception. You must complete this form and leave with our receptionist to be signed by one of our doctors.  You may collect this form the next day.
